•Re­cent (less than six months old) pass­port-sized pho­to on white lower back­ground (with­out spec­ta­cles)
•Couri­er con­fir­ma­tion sheet
Doc­u­ments for age in­ter­view waiv­er ap­pli­cants un­der the age of 14:
•DS-a hundred and sixty Con­sular Ap­pli­ca­tion con­fir­ma­tion page with bar code
•Pho­to­copy of Sco­tia Bank re­ceipt
•Ap­pli­cant’s cur­lease skip­port with as a minimum of six months of re­fundamental­ing va­lid­i­ty
•If ap­plic­a­ble, the child ap­pli­cant’s skip­port with maximum re­cent US visa
•Re­cent (much less than six months antique) bypass­port-sized pho­to on white returned­floor (with­out spec­ta­cles)
•Couri­er con­fir­ma­tion sheet
•Pho­to­replica of the ap­pli­cant’s delivery cer­tifi­cate
•Pho­to­copy of the par­ent’s valid US visa(s)
•Parental con­despatched permit­ter if one of the par­ents does no longer have a legitimate visa and/or is not trav­el­ling
•Pho­to­reproduction of the nation-trav­el­ling par­ent’s iden­ti­fi­ca­tion card or bypass­port.
•DO NOT SEND orig­i­nals of any civ­il document­u­ments
Doc­u­ments for age in­ter­view waiv­er ap­pli­cants over the age of seventy-nine:
•DS-160 Con­sular Ap­pli­ca­tion con­fir­ma­tion page with bar code
•Pho­to­copy of Sco­tia Bank re­ceipt
•Ap­pli­cant’s cur­rent skip­port with at least six months of re­important­ing va­lid­i­ty
•If ap­plic­a­ble, ap­pli­cant’s skip­port con­tain­ing the most re­cent US visa
•Re­cent (much less than six months vintage) pass­port-sized pho­to on white again­ground (with­out spec­ta­cles)
